Waiver and Release
In consideration of the contemplation of participation of the undersigned as a volunteer and /or participant in the Pencak Silat Martial Arts program (the “Program”), the sufficiency of which is hereby acknowledged, the undersigned HEREBY RELEASES, WAIVES AND FOREVER DISCHARGES Pencak Silat Alazhar SBD Martial Arts, Pencak Silat Alazhar Canada, its associates, and its employees and volunteers, all hosts, sponsors and contributors, and all other associations, sanctioning bodies and sponsoring companies, and all their respective agents, officials, servants, contractors, representatives, elected and appointed officials, successors and assigns (the “Released Parties”) OF AND FROM ALL claims, demands, damages, costs, expenses, actions and causes of action, whether in law or equity in respect of death, injury, loss or damage to the undersigned’s person or property HOWEVER CAUSED, arising or to arise by reason of the undersigned’s participation in the said Program, AND NOTWITHSTANDING that same may have been contributed to, or occasioned by, the negligence of any or all of the aforesaid Released Parties.
IT IS FURTHER ACKNOWLEDGED that the undersigned assumes all risk of personal injury, loss of life or property loss resulting from any cause whatsoever, including but not limited to the inherent risks of the activities which the undersigned may take part in while at the Program, or any activities that take place at the Program.
THE UNDERSIGNED AGREES THAT HE/SHE HAS READ AND UNDERSTANDS THE AFOREMENTIONED WAIVER. THE UNDERSIGNED FURTHER REPRESENTS AND WARRANTS THAT HE OR SHE IS OF LEGAL AGE OF MAJORITY FOR THE PROVINCE OF ONTARIO, AND IS PHYSICALLY FIT TO ASSIST/PARTICIPATE IN THE PROGRAM.
